Connects and Disconnects - All new customers must apply for telephone service in person at our business office at 611 Second St.  Customers applying for reconnects and disconnects must contact our business office at 424-2345 or in person at 611 Second St.  Applicants must pay all previous indebtedness to the Cooperative for prior telephone service before service will be connected.  Payment of deposits and service charges may also be required.  Requests for subsequent changes to service and/or equipment may be required in writing, as well as payment of additional deposits and/or service charges.  All telephone connects and disconnects shall be made by Cordova Telephone Cooperative Personnel only.  If you doubt persons representing themselves as employees of the Cooperative, call 424-2345 and ask the customer service representative to verify employment.

Your Number is Important- There is no other telephone number exactly like yours in the United States or Canada when it is preceded by the area code.  The Cooperative will use every effort to avoid changes of your number, but reserves the right when necessary in the conduct of business to change the telephone number.

You are Responsible- you are responsible for all calls placed from your phone.  This includes long-distance, as well as modem and FAX calls, regardless of who places the call or who might accept collect calls.  We can place blocks on your telephone line to block collect calls (this does not include international calling), third party calls, or all long distance calls dialed from your phone.  We can also allow only calling card and collect calls out from your phone.  For more information, contact the business office. 

Business and Residential Service- Determination as to whether a subscriber's service should be classified as business or residence will be based upon the character of use per the telephone tariff, regardless of type of premises.  A BUSINESS is where use is primarily or substantially of a business, professional, institutional or otherwise occupational nature.  If your phone number appears in advertising, you will be charged a business rate. A RESIDENCE is where use is primarily of social or domestic nature.

Ownership and Use of Equipment-   Equipment may be leased or purchased from the Cooperative or provided by the customer.  Leased equipment shall be and remain the property of the Cooperative, whose agents and employees have the right to enter said premises at any reasonable hour for the purpose of installing, inspecting, or repairing the instruments and lines, and (upon termination of the service) for the purpose of removing such instruments that may be the property of the Cooperative.
